Lobby Supervision
RequiredRequired for ALL students. If you have multiple students, please complete one form per student. Details on unaccompanied student policies are included in the form.
Student Participation Waiver
RequiredLets your child participate in MSDA's in-person classes and events while acknowledging the associated risks.
Unsupervised Dismissal Release
For parents of children 11 and older who wish to authorize their child to leave MSDA on their own.
Bring a Friend Waiver
Allows a guest to participate in an MSDA class during Bring a Friend Week.
Withdrawal Notice
Use this form to officially notify MSDA that you wish to withdraw from the studio.
E-Payment Removal
Use this form to decline having a bank or credit card automatically drafted for tuition payments.
Service Society Application
Apply to participate in MSDA's Service Society program. Please submit at least 2 weeks before your intended start date.
